SIN document requirements
You need an original primary document proving identity and status (such as a birth certificate, citizenship certificate, PR card or a work or study permit), a secondary document if your name differs, and proof of address. Photocopies are not accepted in person.

Name differences
If the name on your primary document differs from the one you use, bring a secondary document such as a marriage certificate or legal change-of-name document.
